The information in this enrollment request is complete and accurate to the best of the student's knowledge.
SafePass must confirm instructor availability, pickup area, schedule, and any road-test vehicle request before service is confirmed.
This form records a preferred deposit or payment amount. It does not collect money or prove payment. Nadi will provide the approved payment instructions.
Any remaining balance is due at the time stated in the package summary or final written quote.
SafePass's published safety, privacy, and BDE completion policies apply.
SafePass uses the submitted information only to administer the requested training, meet driving-school record requirements, communicate with the student or guardian, and maintain the agreement record. The full record is kept in restricted storage; ordinary notification emails omit date of birth, home address, gender, and signatures.
Driving instruction cannot guarantee a test result, licence, insurance discount, or course completion without the student's required participation.
The typed names below are intended as electronic signatures for this enrollment request.
Before SafePass activates the student's online course access, BDE payments are refundable less the $50 administrative fee. After online access is activated, the first 50% of the total contract price shown in the signed package summary, including the administrative fee, is allocated to online-course access and administration and is non-refundable. The refund is the lesser of the amount paid above that 50% allocation after any used included road-test vehicle service is deducted, or the value of unused in-car instruction at the regular non-package rate of $50 per 45 minutes, prorated for partial time. An included local road-test vehicle service already used is valued at $190. A refund cannot exceed the amount paid or be less than $0. For this package, the 50% allocation is $524.50. Lessons cancelled or rescheduled with less than 24 hours notice, and missed lessons, are charged in full and treated as used when a package refund is calculated. Lessons cancelled at least 24 hours in advance may be rescheduled or refunded.

BDE students must complete the required online, home-assignment, and in-car components and meet applicable MTO course requirements before SafePass can record completion. Course access, lesson scheduling, and completion remain subject to the student's participation and SafePass confirmation.
